Sleep Disorders Medicine

The field of sleep medicine is relatively new and composed of physicians from a variety of medical and surgical specialties. The primary professional organization for the field is the American Academy of Sleep Medicine (AASM). Dr. Rich has remained in good standing with the AASM and our Portland location is AASM accredited, ensuring patients that care in our center is of the highest quality. Certification of sleep medicine physicians has been through both the American Board of Sleep Medicine and more recently through individual specialty boards. The American Board of Internal Medicine is responsible for administering certification exams for the specialty of Sleep Medicine with certification being appointed by the physician’s specialty board. Dr. Rich is a board certified diplomat of the American Board of Sleep Medicine and the American Board of Psychiatry and Neurology. A variety of other research organizations focus on sleep and its disorders. The importance of this field is receiving ever-widening recognition at all levels of government and industry alike.

Why talk to insomnia patients about sleep evaluation or testing. Most patients with insomnia will respond favorably to hypnotic medications or behavioral changes. Failure to improve from first line treatments for insomnia should raise the concern for insomnia secondary to a sleep fragmenting disorder such as sleep disordered breathing (sleep apnea) or periodic limb movement disorder. The likelihood of secondary insomnia due to these conditions is increased in patients with sleepiness, non-restorative sleep, restless leg symptoms, a large neck circumference (> 16” in women, 17” in men), crowded upper airway, morning or nocturnal headaches or more conspicuous symptoms such as snoring or disturbed breathing during sleep.
